Free Custom Mouthguards — Fremont Athletics

Dr. Robert Yee, DDS (Sunnyvale) is providing free custom-fitted athletic mouthguards for our student-athletes — normally up to a $250 value. Sign up below for an impression appointment window at his office. Impressions take about 30 minutes. Questions? Call/text the office at (408) 733-4473.

What happens after you sign up:
  1. You'll get a confirmation email at the address above with a link to the required consent form on DocuSign (also linked below).
  2. On DocuSign, you'll first enter just your name and email — then the consent form opens for you to fill out and sign. It takes about 2 minutes.
  3. You'll see Dr. Robert Yee and AD Daniel McCune already listed on the form — that's expected. Both automatically receive a copy when you finish. There's nothing to print, send, or fax.

I understand a signed parent/guardian consent form is required before impressions can be taken, and I will sign it electronically on DocuSign (or return the paper version) before my athlete's appointment. *
